Nurse Coordinator – Pre/Post Lung Transplant

Lung transplant is the most advanced treatment for patients with severe diseases of the lung, such as emphysema, cystic fibrosis, pulmonary fibrosis and pulmonary hypertension, to name a few. U.S. News & World Report named UCSF Medical Center’s Lung Transplant Program a specialized center of excellence for treating cystic fibrosis and pulmonary hypertension.
Our lung transplant program has performed more than 600 transplants, mostly bi-lateral, since the program began in 1991. The transplant program has established a reputation for accepting and transplanting challenging, complex patients. Despite this, the survival rate for patients receiving lung transplants at UCSF is higher than the national average, according to data compiled by the Scientific Registry of Transplant Recipients.
The Post/Pre Lung Transplant Coordinator provides direct care to short and long term pre and post Lung Transplant patients with multiple, often complicated acute and chronic medical problems and continuity of care. Primary coordinator responsibilities will be to coordinate multiple post transplant, in hospital, and outpatient services, to coordinate efforts of the Transplant Surgery Team, Consulting Services, and the nursing care as pertains to the Lung transplant patient. The secondary responsibility will be to coordinate the work up, listing and monitoring of the pre transplant population in conjunction with the primary pre transplant coordinators. Additional responsibilities include providing ongoing education to the transplant patient (including patient’s family and primary care giver), both inpatient and outpatient, and working with re-admitted patients to deal with ongoing medical issues.
-Active California Registered Nurse License
-Active American Heart Association CPR certification
-5 years or more of RN employment experience in cardiopulmonary critical care
-The flexibility to orient and work at all UCSF Medical Center locations
-Previous transplant coordinator experience
